Marriage

• Fill out a complete Marriage Application
• Original Marriage Certificate plus two copies
• Husband/Spouse Lebanese ID Card or a recent Individual Civil Registry Record (Ikhraj Kayd) plus first three pages of his passport.
• Spouse’s Birth Certificate ( If she is not Lebanese) plus a copy of her passport.
• Copy of Green Card or US Passport.
• Self Stamped return address envelope.

Note: Please provide 2 copies of each original copy you send.

Divorce

• Fill out a complete Divorce Application
• Original Divorce documents( Final Decree) + two notarized copies translated into Arabic language.
• Husband/Spouse Lebanese ID Card or a recent Individual Civil Registry Record (Ikhraj Kayd) or Family Civil Registry Record plus a copy of the passport.
• Spouse’s Birth Certificate, plus one notarized copy ( If she is not Lebanese).
• Copy of Green Card or US Passport.
• Self Stamped return address envelope.

Birth

• Fill out a complete Birth Application
• Original Birth Certificate plus two copies.
• Husband/Spouse Lebanese ID Card or recent Family Civil Registry (Ikhraj Kayd ). Spouse’s Birth Certificate (If she is not Lebanese ) plus copies of both passports.
• Lebanese Marriage Certificate if available plus one copy.
• According to the new international regulations, we can no longer add any children to either parent's passports. kindly, apply for a new passport for your children.
• Copy of Green Card or US Passport.
• Self Stamped returned address envelope.
  Note: Please provide 2 copies of each original copy you send.

Death

• Fill out a complete Death Application
• Original Death Certificate plus two copies
• Deceased Lebanese ID Card or a recent Individual Civil Registry Record (Ikhraj Kayd) or Family Civil Registry Record plus original passport to cancel.
• Self Stamped return address envelope.
  Note: Please provide 2 copies of each original copy you send.

Citizenship

• 2 copies of a recent family civil registry record ( Ikhraj Qayd)
• Notarized statement from the wife requesting the Lebanese citizenship. Spousal approval should be included in the same statement
• 2 copies of the wife's Birth Certificate
• 2 copies of the wife's passport showing the change in her last name
• 2 copies of the Lebanese passport of the husband
• 2 copies of the Marriage Certificate
• $20.00 money order payable to the Consulate of Lebanon
• Self-addressed stamped envelope
